You may use the form below to request an appointment. Appointments are NOT confirmed unless you have been notified. Please allow 72 hours for confirmation.
Our hours are by appointment only between the hours of 10pm and 6pm, Tuesday through Friday. Walk-ins are not currently being accepted.

What to Bring to Your First Visit
- Please download and fill out our intake form. If you do not have access to a printer, you may fill it out at the office before your visit. Our form is thorough, so if you are filling it out before your visit, please allow 10-15 minutes to do so before the start of your appointment time.
- If you are eligible for the First and Last Friday Program, please download and fill out our application and bring with you the 2 forms of proof of income requested.
- If you are taking any medications or vitamins/herbs/supplements, please bring the bottles in with you so we can review exactly what dosages you are taking.
- If you have any copies of lab/blood work done in the last year, please bring that paperwork with you if it's available. If not, we can always sign a release to have the ordering physician fax it to me with your permission.
How Long is a Visit?
- New Patient Visit - 1.5 - 2 hrs, depending on complexity
- Follow Up Visit - 15 min - 1 hr, depending on complexity
- Injection - 10 min
- Blood Work - 10 - 15 minutes
- Physical - 1 - 1.5 hrs, depending on complexity
- Bodywork - 15 - 30 minutes
Your confirmation will include an approximation of how long your appointment will be.

- Bare Medicine will either confirm your requested time, or if that time is already booked, we will suggest the next available appointment time.
- You will hear from us within 72 hours.
- If your requested time is not available, you will have 48 hours to accept or deny the next available time.
- There is a cancellation fee of $90 if cancellations are given less than 24 hours before your appointment time.
